To refer a patient with skin cancer for Mohs or dermatologic surgery, please fill out the 'Consult Fax Sheet' listed below. Please include the pathology report and the patient’s address, phone number(s) and insurance information to (913) 825-3720.

Our office staff will contact the patient to arrange an appointment. Patients will receive a brochure describing Mohs surgery, the process, what to expect and detailed directions to the our nearest office. We will follow patients through their healing process and have them return to the referring provider for the rest of their dermatologic follow-up and care.
